Practicum Goals: This is a list of knowledge or skills you want to achieve or improve during your Practicum.
Supervisor's Statement: A short statement written by your supervisor. This statement should state the agreement to supervise you, his/her signature with credentials. This must be faxed to my home office or sent US mail to Canyon College, Attn.: Your college instructor's name.
A Site paper: A paper telling about your agency or organization, including the origin, funding, purpose, the role within the community and your role as a Practicum student while there. This can be submitted via email, faxed to my home office, or sent as an attachment with an email.
Begin a Personal Journal: This journal is for you to record in a daily your experiences, feelings, or whatever you would like to write about regarding your Practicum. These 5 points are "Honor Points." You do not send your journal in or have to show them to anyone. They are your private recordings. I will automatically add the 5 points to each stage unless you advise me that you are not doing a journal. There are no grades for the journal exercise.
Begins after you notify me of your placement

Stage II:

5 monthly points
(51-400 hours)
Journal - You do not turn the journal in. It is yours to use.
Case Studies: This is a short paper to be done every month during your Practicum. Each paper may be a progressive report on one client, family or group OR, you may submit a different case study situation each month.

I would like an introduction (any name, age, gender, occupation, issues, economic status, history or any information you feel needed to help clarify the study. The study may be a real case or a fictional case.)

I would like a different technique or method of counseling used in each case study if possible.

I would like the Case Study reports/papers to be done in P.I..E. format.
P - Problem - why you are seeing the client(s) and how the problem is effecting the client(s)

I - Intervention -What you are doing? what you are having the client(s) do? Is there anyone else involved (police, spouse, family, job coach, etc.)? Were referrals made? What counseling technique or method are you using? Why you are using the particular intervention? How will you implement it? What are the strengths and weaknesses of the intervention plan?

E - Evaluation - evaluate the method of intervention; the roles & outcome of any others involved with your intervention or client, and evaluate the benefit, if any, the intervention had on the client or situation.
This paper should not exceed two (2) pages. If you do have a format at your Practicum site which includes the information you may use it rather than the P.I.E. format.
