1st Paper: 20 points
Select two (2) clients or hypothetical clients. (1) having a severe mental illness and (1) with a physical impairment. They can be from any age group and either gender. Write up an intake, assessment and goals (no more than 3) that the client has. Who will you contact and how you will implement the plans for these individuals. Include the strengths & weaknesses of the client; what are the barriers and what are your ideas to overcome the barriers? Organize the step you will take and supports you will use. This paper should be 3-6 pages. Use your text and case studies to develop these objectives. Keep a copy of this to use in the 2nd paper.

2nd Paper: 20 points
Review your 1st paper. Using the same clients in your 1st paper evaluate your plan and write each step as a success/failed. What was successful and what wasn't successful?
